Please note that the tables and figures were scanned from a photocopied document so they are not particularly crisp or clear. When the numbers or text were indecipherable, the scanned images have been slightly altered to enhance legibility. The maps (Figures 1-28) were scanned in two resolutions: 75 dpi for viewing via the Web, and 180 dpi for printing crisper images.
The pagination of the printed document was retained.
